Editing a Patient or Caregiver Measures Responses

Patients may enter information incorrectly when completing measures or skip questions which will result in an invalid score. 

Below how to edit responses in Measurement-Based Care.

To edit patient responses to measures in CoCM: 

  1. Click into the Patient Report

  1. Click the relevant score

  1. Click pencil icon

  1. Make necessary change

  1. Click "Save" or "Save & View Report"

To edit patient responses to measures in MBC: 


  1. On the Patient Details page, select Details on the session you would like to input data for.

  1. Select "Enter Data In Bulk". If the measurement window has closed, follow these instructions to re-open the measurement window.

  1. Enter in data and press "Save" at the top.
