How to Re-enroll Patients

Patient's can undergo multiple enrollments. When a patient is unenrolled, they are no longer actively participating in a specific care program and, therefore, will no longer receive measures. A patient will re-enroll when they restart or resume participation in care workflows. Follow these steps and take note of the criteria for re-enrollment:


  1. Go to the patient's Detail Page via the Dashboard or Patient tab.
  2. If the patient is currently unenrolled, it will specify this status in the top left hand side of the patient's Detail Page.
  3. Scroll down and look at the left panel titled "Enrollment History". This documents every enrollment for this individual patient. Select the "Details" for the plan you wish to re-enroll the patient in.
  4. This will bring you to a page containing details for the specified enrollment. On the top left-hand corner, click the button that states "Reactivate Enrollment".
  5. A screen will surface; select "Reactivate".

Re-enrollment Requirements

If the option to re-enroll a patient is not available, this is often because the past enrollment must have been discharged either the current month or last month. This is evident if the "Reactivate Enrollment" button is disabled and grey (as shown below). If this is the case, the best course of action would be to create a new enrollment for this patient.
