Collaborative Care - Phases of Care - Configuration Guide
What are phases of care?
A phase of care represents an individual stage of each patient’s journey. These could include (but are not limited to) things like:
- Referral and Intake
- Active Treatment
- Relapse Prevention
Each phase likely has different workflows, tasks, and expectations for care associated with it. For example, you may do much less patient contact in ‘Relapse Prevention’ than in ‘Active Treatment’.
In our system, each phase is represented by a colored icon. Your organization can configure whatever phases of care they like and give each one a separate icon. By default, the ‘Active Treatment’ phase looks as follows:
How do I configure phases of care?
- Select “Configuration” -> “Phases Of Care”
The order you see the phases of care is the order they will appear when listed in the rest of the tool.
- To change the order, click the the arrow icons to move the phase of care up or down the list.
- To edit a phase of care, click the pencil icon
- Each card contains several elements:
- The name which will be displayed
- The category can be set to match a set of existing categories similar to those provided by the AIMS center which can be used for analytics purposes later.
