Issue with Locating Patient Record - Troubleshooting Guide

The following guide will detail the steps needed to take in order to investigate why a patient record cannot be located.


Basic Troubleshooting Steps:

  1. Has the patient completed an assessment? In some instances, a patient might not appear on the relevant patient search list until they complete an assessment for the first time.
  2. Have the proper care team roles been assigned in the EHR? If those fields aren't clearly detailed in the EHR, then Mirah will randomly assign this field from the list of clinicians related to this patient. If you would like to have a different clinician assigned to a different client, that change will have to be done manually.
  3. Are there duplicate records? There are some cases in which there are two patient records for the same patient, which may cause the system to assign data incorrectly to the first record it finds. To have duplicate patients deleted, contact support@mirah.com
