Collaborative Care - How to add a related person

What is a related person?

A related person is typically a caregiver, parent or teacher that is related to a pediatric patient. For certain measures, surveys are sent to the patient and/or the related people.

*Note - to add a related person in Measurement Based Care (MBC), follow the instructions here.

Navigate to a patient record

1. After logging into Mirah, click on the “Patient Search” tab at the top of the screen
2. Click on the Patient Name to navigate to their record. You can use the search filters to narrow the search results on the patient list page

How to add a related person to a patient

Once you are on a patient page you will have the ability to add a related person.

1. Click on the “Related People” tab on the patient’s record
2. Click on the “Add a Related Person” button on the right of the screen

3. Fill out the related person details. The following fields are required to create a related person:
   1. Last Name
   2. Relationship
4. Click “Save”
