Designating or Updating a Primary Clinician/Care Team

Designating a Primary Clinician:

If a client is enrolled in multiple programs and having sessions with multiple clinicians, Mirah needs to know which clinician is going to be the primary clinician who is using Mirah with that client. Even though every clinician on a treatment team can see the Mirah reports, the client and caregiver will be answering questions about sessions and it can be confusing to them if they aren’t always being asked about the same clinician.

The decision about which clinician will be the Mirah clinician is up to the treatment team. The primary clinician will be the focus of the questions that clients and caregivers answer.   

Updating a Primary Clinician/Care Team

You can change or add-on to a client's Care Team. The Primary Clinician is the clinician responsible for reviewing the client's feedback report.  Additional clinicians can be added to the Care Team under the Primary Clinician. The clinicians on the care team team are linked to episodes of care for clients instead of directly to the client.

Instructions to designate and update Primary Clinician

  1. Go to the client's page in Mirah and select "Edit Demographics".

  2. Scroll down to Primary Clinician. If the clinician listed in that box is correct, there is no further action needed. If you would like to select a different clinician, click the carrot to the right of the clinician's name. 


  3. A list of clinicians from your practice will appear. Select the clinician that will be designated as Primary Clinician. 


  4. The new clinician's name will now appear in the Primary Clinician box. Hit submit to save.

  1. The update Primary Clinian name will appear on the client page as the first name within Care team: with the title registered with that clinician user e.g Psychiatrist.



Instructions to designate and update Care Team

  1. On the client page, scroll down below the sessions to the Active Episode of Care and click "Details".

  1. Edit information for this episode of care by clicking "Edit".

  1. Add members to the care team by clicking the providers dropdown and selecting a provider. Reduce providers that appear by clicking "Show providers from this unit" to see the smaller subset of providers.

  1. Select the relationship to the client in the "Relationship" dropdown and press the "+" to add them.

  1. Added members will appear on the care team list.

  1. Press "Save" at the bottom to save changes.

  1. Note that even after changes are made, the platform will not automatically leave the edit screen. To return to the episode of care screen, click the client name or episode of care name.

  1. Added members of the care team now appear on the client page and the episode of care page with the primary clinician first.

  1. Remove members of the care team by clicking the "-" on the Edit episode of care screen.

  1. Save changes and return to other screens in the same way as described above.
  2. The removed members will no longer appear on the client page and the episode of care page.

  1. Note that if the Primary Clinician is removed, the longest time member of the care team will automatically be made the Primary Clinician. If there are no remaining members of the care team, the client will not have a Primary Clinician.
