A compiled report featuring completed questionnaires from a variety of respondents, and scored appropriately. See Questionnaire Response for the individual respondent breakdown, Observation for scored values (e.g. the PHQ-9 summary score) and Answer for individual item responses.

| effectiveDate | Effective Date | timestamp | 0..1 | Represents the best guess at the time at which the information was most current. This is defined as the completion date of the first questionnaire to be completed. This will be blank while the survey is registered, and will be set when the report is canceled, or at least one questionnaire has been completed. Currently the system may assign effective dates for canceled reports, when the report was canceled. In certain circumstances, if no respondent completed a survey, it may be necessary to look at the target date of the encounter to estimate a date. |
